First Visit:

  • For your first visit, please arrive 15 minutes early in order to fill out paperwork. You may also print and fill out the above form and bring it with you.

  • Please bring or wear loose comfortable clothing that can be pushed above the knees and elbows, ideally shorts and a loose fitting tank top. If clothing restricts access to appropriate acupuncture points, draping will be provided to maintain privacy.

  • Avoid coming in on an empty stomach; be sure to eat within a few hours of your appointment.

  • We look forward to seeing you and if you have questions or need directions, don't hesitate to call 509-675-0062.

 

Insurance:

Beginning October 5, 2023, Colville Acupuncture will only be in network with VA Care in the Community insurance. If you are a veteran and would like acupuncture treatment, please ask your primary care provider to request acupuncture services with Colville Acupuncture in order to generate a new authorization.

We are sorry to announce after October 5, 2023, we will no longer be in network with all private insurance. This includes Premera, Regence, Blue Cross Blue Shield, Aetna, Unified, Lifewise, Molina, GEHA, and all other private and government insurance including Medicare, Medicaid, and plans from the WA State Health Benefit Exchange.

All visits are to be paid in full at time of service (except VA authorized visits) beginning October 5. After that date, a superbill will be provided to patients who have acupuncture benefits and would like to receive reimbursement for their visits from their insurers. Please contact your insurer to ascertain your out of network benefits, reimbursement rate, and how to submit a claim.

Health Savings Accounts (HSA):

Acupuncture is an IRS qualified health care expense for Health Savings Accounts (HSAs).  If you plan to use your HSA to pay for your treatments, let me know at the time of treatment and I will happily give you a superbill receipt.

 

Colville Acupuncture has a 48 hour cancellation policy:

If you need to cancel or reschedule your appointment, kindly do so at least 48 hours in advance of the appointment out of respect for the practitioner and other patients who could use the time slot.  No show appointments or very late cancels will be charged a fee of $30. Patients who no show or cancel at the last minute repeatedly may be denied the opportunity to reschedule. Thank you!
